  • A brief description of your supplies or services,
  • Your physical and remittance addresses,
  • Phone numbers for ordering and accounting,
  • An email address where purchase orders can be sent,
  • Your website,
  • Contract numbers if you are under any cooperative purchasing agreements (i.e. Mohave, SAVE, AZ State, GPPCS, 1GPA, etc.) and
  • Your current W-9 form.
