Homebound Services

Purpose

Homebound Services are provided to CGUHSD students who cannot attend school due to an acute illness, disease, accident, or other health condition for no less than three months.

Qualifications

All students referred to and applying for Homebound Services must first be found eligible for the program. The student must be examined by a competent medical doctor (D.O. or M.D. certification) who certifies that the student cannot attend regular classes for at least three months.

Application Process

Parent and Doctor complete Homebound Application
  • Doctor must have seen student within 30 days of applying for services
  • Doctor must indicate diagnosis and reasons why student cannot attend school
  • Doctor must state that student will be out of the classroom for no less than three school months
  • Doctor must indicate an anticipated date of when the student is expected to return to school
  • CGUHSD Homebound Coordinator will contact doctor to verify information, if necessary
  • CGUHSD Homebound Coordinator interviews family/student
  • 504 or IEP Meeting may be held to discuss eligibility and services

Enrollment and Services

A certified teacher(s) will provide up to four hours per week in core subjects. Direct instructional hours of service include academic and special education related services (if needed). Services will be during school hours or after school hours based on teacher availability. Please contact the CGUHSD Homebound Coordinator for further information. 

ARS § 15-901

ARS § 15-901: "Homebound" or "hospitalized" means a pupil who is capable of profiting from academic instruction but is unable to attend school due to illness, disease, accident, or other health conditions who has been examined by a competent medical doctor and who is certified by that doctor as being unable to attend regular classes for a period of not less than three school months or a pupil who is capable of profiting from academic instruction but is unable to attend school regularly due to chronic or acute health problems who has been examined by a competent medical doctor and who is certified by that doctor as being unable to attend regular classes for intermittent periods of time totaling three school months during a school year. The medical certification shall state the general medical condition such as illness, disease, or chronic health condition; that is why the pupil cannot attend school. Homebound or hospitalized includes a student who is unable to attend school for a period of less than three months due to a pregnancy if a competent medical doctor, after an examination, certifies that the student is unable to attend regular classes due to risk to the pregnancy or to the student's health.
